Host command execution via fail-open batch approval

Missing authorization in Hermes Agent 2026.4.0 through 2026.4.16 allows remote attackers to execute host commands via a crafted JSONL dataset. The Batch Runner leaves approval-context variables unset, so check_all_command_guards returns approved before dangerous-command checks. The attack requires an operator to process attacker-controlled dataset prompts; commands then run with the Hermes Agent process privileges.
