Kernel crash via unbounded NVMe PRP list

Out-of-bounds write in the Linux 6.17 megaraid_sas driver allows local users to crash the kernel through large NVMe I/O requests. The megasas_make_prp_nvme() loop writes PRP entries into cmd->sg_frame without checking instance->max_chain_frame_sz, so a 4 MiB transfer can overrun the DMA-pool buffer by one page. Reachability requires an NVMe device behind a MegaRAID controller whose firmware reports a large MDTS; a mapped following page can instead corrupt another in-flight command's PRP list.
