Stale IRQ handlers via incorrect rollback indexing

Resource cleanup error in the Linux kernel pm8001 SCSI driver allows attackers to retain MSI-X handlers via unspecified vectors. The pm8001_request_msix rollback loop supplies the failed vector's index to free_irq() on every iteration rather than each registered vector's index and matching dev_id. Reachability requires a partial request_irq() failure during MSI-X setup, and public sources document no consequence beyond stale handlers.
