Kernel heap disclosure via uninitialized diagnostics fields

Information disclosure in the qla2xxx SCSI driver in Linux 4.8 and later allows local users to read kernel heap data via D_Port diagnostics. qla2x00_do_dport_diagnostics() allocates a nonzeroed response, partially fills it from the request, then copies uninitialized options and unused fields to user space. Reachability requires local access to the qla2xxx D_Port diagnostics interface and compatible Fibre Channel hardware; exposure is limited to residual kernel heap contents.
