Command duplication or loss via unlocked NVMe rejects

Race condition in the Linux kernel 6.6 qla2xxx driver allows attackers to duplicate or drop commands during concurrent NVMe-FC activity. Two callers invoke qla_nvme_ls_reject_iocb without holding hardware_lock, allowing request-ring allocation and doorbell updates to race with normal I/O. Exploitation requires the qla2xxx NVMe-FC path and concurrent reject and submission activity; corrupted producer state can misdirect storage operations.
