Adapter hang via unsynchronized firmware-state query

Race condition in Linux allows local users to hang a QLogic adapter by reading firmware state from sysfs during chip recovery. qla2x00_fw_state_show() bypasses chip-down and EEH-busy guards by issuing a second firmware-state mailbox command outside optrom_mutex after its guarded path fails. Reachability requires a qla2xxx-supported adapter and a sysfs read during ISP reset or PCI error recovery; the resulting hang interrupts adapter service.
