Firmware reset disruption via unguarded LS reject

Improper state validation in Linux 6.6 and affected qla2xxx revisions allows attackers to disrupt a firmware reset through an NVMe LS response. The qla_nvme_xmt_ls_rsp and qla2xxx_process_purls_pkt paths emit an LS reject IOCB and write the queue doorbell even when fw_started is clear. The condition requires qla2xxx-managed NVMe/Fibre Channel hardware during a firmware stop, reset, or EEH window; no external trigger or broader impact is published.
