Access-token issuance via unbound refresh tokens

Improper authentication in Data Plane Core Siglet 0.1.0 through 0.1.3 allows remote attackers to obtain access tokens using a stolen refresh token. The refresh handler accepts the bearer refresh token without requiring proof that the requester controls the private key for the issuer DID. Possession of a refresh token is therefore sufficient to mint replacement access credentials for the token's identity.
