Unauthenticated SSRF via environment-gated provider URL validation

SSRF in THU-MAIC OpenMAIC 1.0.0 and earlier allows remote attackers to read cloud metadata via attacker-controlled provider URLs. Provider routes accept x-base-url or baseUrl values, while middleware fails open without ACCESS_CODE and validateUrlForSSRF runs only in production. The default unset access code plus a non-production build exposes cloud credentials and other metadata without authentication.
