Internal data disclosure via unvalidated MCP server URL

SSRF in OGX 1.3.1 and earlier allows remote attackers to read internal service data through the OpenAI-compatible Responses API. The _process_mcp_tool path passes an attacker-controlled server_url to list_mcp_tools, which opens an MCP connection without applying validate_url_not_private. The default starter configuration requires no authentication, exposing internal services and cloud metadata to requests from the server's network position.
