Process crash via quadratic namespace-map cloning

Denial of service in affected xmldom releases allows remote attackers to crash application processes by submitting crafted XML. appendElement in lib/sax.js copies the complete in-scope namespace map for every nested element that declares a new prefix, retaining quadratically many entries on the parse stack. Any application that parses attacker-influenced XML under default options is exposed; the effect is heap exhaustion and process loss, not data disclosure or modification.
