Service hang via unbounded LLM retry loop

Denial of service in Vectify AI PageIndex allows remote attackers to hang PDF indexing with a crafted table of contents. The toc_transformer function in pageindex/page_index.py repeatedly requests continuation after length-truncated LLM output without a retry limit or reachable bailout. Unauthenticated document submission must reach the affected handler; the loop can hold a CPU thread, keep issuing paid API requests, and interrupt the indexing service.
