Unauthorized record disclosure via empty-scenario retention

Incorrect authorization in Ash 3.4.44 through 3.32.1 allows attackers to read records denied by runtime read policies. In Ash.Policy.Authorizer.check_result/1, the empty-scenario branch retains a record after every possible policy scenario has been discarded as impossible instead of marking that record forbidden. The flaw is reachable only when an application evaluates a read policy at runtime, and it exposes denied records to any actor that can invoke the affected read path.
