Record overwrite via duplicate primary-key creation

Improper access control in Ash 0.4.0 through 3.32.1 allows attackers to overwrite ETS or Mnesia records through create actions. The create routines use keyed inserts that replace an existing primary-key entry without an existence check, bypassing update actions and policies. Exploitation requires an application to expose a create action with a caller-controlled primary key on an ETS- or Mnesia-backed resource, and it destroys and replaces the targeted record.
