Access-control bypass via raised change-step guard

Fail-open handling in Ash 3.0.0-rc.17 through 3.32.1 allows attackers to bypass gated changes via malformed action input. Ash.Reactor.ChangeStep's apply_validation/3 rescues a raised where-guard exception as a normal validation error, and apply_where_clauses/3 treats it as an unmet condition and skips the change. Reachability requires an application to pass attacker-controlled values to a guarded Reactor change; the skipped operation can preserve unauthorized privileges or omit tenant scoping or hashing.
