Input-validation bypass via skipped outer array constraints

Improper input validation in Ash 2.16.1 through 3.32.1 allows attackers to persist nested arrays that violate declared outer constraints. Ash.Type.apply_constraints/3 maps only inner-array constraints over each element and omits list_constraint_errors/2 for the outer array, so length and nil-item rules are skipped. Reachability requires an application to expose a doubly nested array attribute or action argument; accepted invalid data can affect integrity, while a nil value can terminate the request.
