Cross-tenant access via nil authorization tenant

Incorrect authorization in AshPhoenix 2.1.26 through 2.3.24 allows remote authenticated users to access other tenants' LiveViews. SubdomainHook calls handle_subdomain during on_mount before handle_params assigns the request tenant, so the callback always receives nil. Exploitation requires an application to use this callback as its tenant authorization gate; a permissive nil branch grants cross-tenant access, while other handlers can crash.
