Account restriction bypass via unchecked API-key middleware

Authorization bypass in Gophish 0.12.1 and earlier allows remote authenticated users to retain API access despite account restrictions. RequireAPIKey in middleware/middleware.go resolves a user from an API key and invokes the requested API handler without checking AccountLocked or PasswordChangeRequired. A valid API key is required; the same user can also clear either flag through PUT /api/users/{id}, restoring web access after an administrator imposes the restriction.
