PCF crash via stale HTTP/2 stream assertion

Denial of service in Open5GS 2.7.0 through 2.7.7 allows remote attackers to crash the PCF via a timed-out SM policy request. A delayed NRF response enters the fallback path after ogs_sbi_stream_find_by_id returns NULL for the closed HTTP/2 stream, and an assertion terminates PCF. An SM Policy Association for a VPLMN SUPI must trigger delayed BSF discovery, and the requester must disconnect before the response arrives.
