Cross-course quiz tampering via unchecked question identifiers

Broken object-level authorization in LearnPress before 4.4.6 allows remote authenticated users to modify quiz content through answer insertion. The question-answer insertion path accepts an arbitrary question identifier without verifying that the Instructor owns the referenced question. Instructor access is required, and successful requests persistently add answers to quizzes in courses owned by other instructors.
