Second-factor bypass via client-keyed OTP lockout

Authentication bypass in miniOrange 2FA 6.2.8 through 6.3.0 and 18.0 through 19.2 allows remote authenticated users to defeat OTP checks. The attempt limiter keys failures to a client-supplied identifier rather than the targeted account, and a second OTP validation endpoint applies no limit. Knowledge of the victim password is required; changing the identifier permits unlimited OTP guesses and eventual account access.
