Persistent access via skipped adjacent role removals

Improper authorization in ZITADEL before 4.16.0 allows remote authenticated users to retain Granted Project access after role deletion. During cross-organization cleanup, the role-removal loop skips adjacent entries when administrators delete two or more project roles together. The flaw is limited to User Grants on Granted Projects; retained permissions can preserve administrative or other access that deletion was meant to revoke.
