HTTP request smuggling via unencoded decoded paths

HTTP request smuggling in Reverse::Proxy before 0.04 allows remote attackers to inject upstream requests through crafted URL paths. The rp_target_url and rp_raw_request routines append percent-decoded PATH_INFO bytes without re-encoding, so CRLF terminates the serialized upstream request line. Unauthenticated network reachability is sufficient, and the injected request can access upstream paths hidden by the proxy's routing.
