Insufficient session expiration in Hex.pm 2025-10-10 through 2026-08-23 allows remote authenticated users to retain private-package read access. The refresh grant rebuilds access tokens from stored explicit organization scopes without rechecking current membership. A valid refresh token and prior organization membership are required; after removal, CDN authorization from stale claims extends access for up to 30 days.
