TOTP brute-force via in-memory throttling

Improper authentication throttling in jahlives openssl_encrypt before 1.4.0 allows remote attackers to brute-force TOTP codes across workers. In openssl_encrypt_server/modules/pepper/totp.py, the TOTPRateLimiter stores failed-attempt and lockout state in process-local in-memory dictionaries behind a class-level TOTPService limiter, so workers do not share counters and every restart clears prior state. Exposure requires the server component and TOTP flow to be reachable, and the flaw weakens the online guessing control rather than bypassing TOTP by itself.
