Host denial of service via unbounded IOTLB allocation

Resource exhaustion in the vhost IOTLB subsystem in affected Linux kernels allows local users or guest OS users to crash the host. vhost_iotlb_add_range_ctx() lets non-retiring tables keep allocating map entries after their configured limit instead of returning -ENOSPC. Reachability requires access to a vhost device node or a guest that can drive virtio IOTLB misses; the flaw exhausts host kernel memory without reading or modifying data.
