Kernel memory read-write via unbounded mmap offset

Out-of-bounds access in Linux ALSA usx2y allows local users to read or write kernel memory through a crafted hwdep mmap offset. The snd_us428ctls_vm_fault and PCM hwdep fault handlers add an unchecked page offset to a one-page buffer and map the resulting kernel page read-write. Reachability requires access to the hwdep node of an attached US-X2Y device after loading its FPGA image; an invalid page can instead trigger a kernel oops.
