Local system crash via folded page-table indexing

Out-of-bounds page-table walking introduced in Linux 6.11 allows local users to crash RISC-V systems during memory hot-remove. In arch/riscv/mm/init.c, remove_pud_mapping() and remove_p4d_mapping() add a nonzero address index after folded-level helpers return the parent entry, walking beyond that table and passing an invalid page to __free_pages(). Reachability requires a RISC-V Sv39 or Sv48 system and a memory hot-remove operation; the reported consequence is a kernel crash.
