System crash via unchecked virtqueue mapping index

Out-of-bounds access in the Linux kernel vhost/vDPA subsystem from 5.8 allows local users to crash the system through a crafted mapping. vhost_vdpa_mmap() and vhost_vdpa_fault() treat vma->vm_pgoff as a virtqueue index and pass values below u16 but beyond v->nvqs to driver callbacks. Low-privilege local access to an affected vhost/vDPA device is required; an invalid callback result can drive an invalid PFN remap and crash the host.
