Kernel crash via incorrect DMA cleanup index

Improper DMA cleanup in Linux 5.7 and later allows remote attackers to leak DMA mappings and crash affected OcteonTX systems. The sg_cleanup path in drivers/crypto/marvell/octeontx/otx_cptvf_reqmgr.c indexes list[i] instead of list[j], repeatedly unmapping the failed entry while leaving earlier mappings allocated. Reachability requires Marvell OcteonTX CPT hardware processing fragmented offloaded cryptographic traffic that causes a DMA-mapping failure; repeated triggers exhaust mappings or panic the kernel.
