Unauthenticated provider injection via forged OAuth state

Authentication bypass in Dokploy before 0.29.13 allows remote attackers to insert a GitHub App provider into another organization. The /api/providers/github/setup callback trusts organizationId and userId values in OAuth state and passes them to createGithub, which persists the App secrets under that target. Exploitation requires a GitHub account and a target organization identifier; downstream deployment control requires a legitimate user to select the planted provider.
