Storage I/O hang via unreleased range lock

Deadlock in the Linux dm-integrity target allows local users to hang storage I/O by submitting a bio outside configured limits. The mapping path locks the request range before dm_integrity_check_limits and returns DM_MAPIO_KILL on failure without releasing that lock. Reaching the path requires local low-privileged access and an out-of-limits bio; the unreleased range lock can deadlock later I/O on the mapped device.
