Service interruption via IOACCEL2 reset resource leak

A DMA resource leak in the Linux kernel hpsa SCSI driver allows local users to interrupt service through repeated IOACCEL2 resets. When phys_disk->in_reset is set, the IOACCEL2 path returns without unmapping its SG chain and SCSI command or reducing the outstanding-command count. Exploitation requires low-privileged local access and an hpsa-managed Smart Array controller; accumulated leaks can exhaust DMA resources and stop storage I/O.
