SCSI recovery blockage via incomplete abort cleanup

Resource-management flaw in Linux allows local users to block SCSI I/O recovery when an abort request exhausts request tags. In efct_hw_io_abort(), an efct_hw_reqtag_alloc() failure returns without releasing the io_to_abort reference or clearing abort_in_progress. A low-privilege local user must reach the Emulex Fibre Channel SCSI abort path under request-tag exhaustion; repeated aborts then fail and recovery remains blocked.
