Credential exposure via insecure vault fallback

Credential exposure in SmythOS Runtime Environment 0.0.15 and earlier allows remote authenticated users to steal vault API keys. The adaptModelParams function in packages/sdk/src/LLM/utils.ts silently substitutes ['vault'] when apiKey and credentials are omitted, then sends the resolved global vault token to the attacker-controlled baseURL. Exploitation requires a deployment that lets a user define an LLM baseURL while the Node.js process can access the system vault.
