Unauthenticated server crash via unchecked target dereference

NULL pointer dereference in open62541 1.5.5 allows remote attackers to crash an OPC UA server via a crafted AddReferencesRequest. Operation_addReference rejects targets only when targetServerUri is nonempty, but a nonzero serverIndex also makes the target non-local and leaves targetNode NULL before its browseName is hashed. An anonymous OPC UA session is sufficient, and the invalid member pointer passed to UA_QualifiedName_hash terminates the server process.
