Login CSRF via unbound identity callback

Cross-site request forgery in Livebook 0.15.0 through 0.18.6 and 0.19.0 through 0.19.8 allows attackers to bind victim sessions to attacker identities. The Livebook.ZTA.LivebookTeams.handle_request/4 callback exchanges an attacker-supplied code and stores its token without checking state or a nonce tied to the initiating browser session. Exploitation requires membership in the same Livebook Teams organization, victim interaction within the code's short lifetime, and Teams identity configuration; resulting work and secrets are associated with the attacker's account.
