Authenticated document-follow creation via missing permission checks

Missing authorization in Frappe Framework before 15.112.0 and 16.23.0 allows remote authenticated users to follow restricted documents. The update_follow path accepted caller-supplied DocType and document names, then created a Document Follow record without enforcing document-read or record-create permission. An authenticated account with follow notifications enabled could alter its own follow state for a restricted tracked document and receive later change notifications.
