One-time request replay via premature slot clearing

Capture-replay in Apache Tomcat's DIGEST authenticator allows remote attackers to repeat one authenticated request. DigestAuthenticator.NonceInfo clears the slot for count modulo windowSize before incrementing count, marking an upper-bound nonceCount unseen again. The gate is a captured valid DIGEST request sent at the replay window's upper boundary before windowSize requests, and only one replay succeeds while that count remains in the window.
