Authenticated code execution via unrestricted decoder library loading

Missing authorization in PostgreSQL logical decoding allows remote authenticated users to execute code as the server operating-system account. The slot-creation path accepts any loadable library named as its output plugin instead of limiting replication roles to authorized decoder modules, causing the server to load an attacker-selected file. Affected branches are before 14.24, 15.19, 16.15, 17.11, and 18.6; exploitation requires REPLICATION privilege and a library visible to the server account.
