Host crashes via special-page PoD reclamation

Memory-state corruption in Xen 3.4 and later allows guest OS users to crash x86 hosts through PoD page reclamation. The PoD reclaim path accepts pages that are not regular guest RAM and reclaims them as ordinary memory, corrupting Xen's memory-management state. Reachability requires an x86 HVM or PVH guest started with PoD, which xl enables when maxmem exceeds memory; crashes or other host malfunctions follow.
