2FA bypass via unauthorized secret regeneration

Authentication bypass in Grav before 2.0.4 allows remote attackers with a victim's password to defeat TOTP-based two-factor authentication. The login plugin's regenerate2FASecret task checks only that the pending session user exists, then saves and returns a replacement secret without authorization or a CSRF nonce. A known password and enabled TOTP are required; exploitation grants the victim's session and permanently replaces the legitimate secret.
