Unauthenticated API access via insecure server defaults

Missing authentication in the LightRAG API server 1.5.4 and earlier allows remote attackers to read and alter data via exposed endpoints. The combined_auth dependency in lightrag/api/utils_api.py returns without checking identity when both AUTH_ACCOUNTS and LIGHTRAG_API_KEY are unset, while the server binds to 0.0.0.0. A network-reachable default deployment exposes document, graph, pipeline, cache, and LLM operations, including deletion and resource consumption.
