Prompt injection via CRITICAL-only blocking

Protection mechanism failure in PraisonAI before 4.6.78 allows remote attackers to pass prompt injections through to the model. The scan_text function blocks only CRITICAL results, while one or two detector-family matches are labeled HIGH and neither blocked nor sanitized. The defense must be explicitly enabled, and successful bypasses can alter model or tool-directed behavior without directly affecting confidentiality or availability.
