Reflected XSS in CRM Perks Contact Form Entries 1.5.2 and earlier allows remote attackers to execute script in a visitor's browser. The leads-page Screen Options path outputs an attacker-controlled $form_id value without the neutralization required for safe HTML generation. No account is required, but a privileged user must open a crafted link or page or submit a form, limiting the documented effect to browser-context script execution.
