Account access via unlimited 2FA code guesses

Missing rate limiting in AIL Framework 6.8.0 and earlier allows remote attackers to gain account access via repeated OTP guesses. The verify_2fa route checked each submitted code without recording failed OTP attempts or enforcing a retry limit. The attacker must first reach the 2FA step, such as by completing password authentication, before unlimited guesses can defeat the second factor.
