Cross-namespace credential revocation via unscoped lease lookup

Incorrect authorization in OpenBao before 2.5.5 allows remote authenticated users to revoke cross-namespace leases using known lease IDs. In vault/expiration.go, FetchLeaseInfo returned cached entries without comparing their namespace to the request context, while loadEntry rewrote the context from the lease ID before reading storage. Access to sys/leases/revoke in any namespace and a foreign lease ID are required; revocation invalidates the lease and its underlying credential.
