Authenticated server crash via prompt-embeds assertion

Reachable assertion in vLLM GPUModelRunner 0.12.0 through versions before 0.24.0 allows remote authenticated users to crash the server. The _init_mrope_positions method asserts that prompt_token_ids is present even when a prompt-embeds-only request legitimately leaves it unset. Exploitation requires prompt embeds to be enabled with an M-RoPE model, and the resulting EngineCore failure shuts down the server application.
