Unauthenticated operator data access via header guard bypass

Improper authentication in Facebook/Meta Ads MCP Server 1.0.113 and earlier allows remote attackers to read or modify operator ad data. AuthInjectionMiddleware accepts any X-Pipeboard-Token as sufficient for its guard but establishes no authentication context, causing tool handlers to reuse META_ACCESS_TOKEN. Exploitation requires network-reachable streamable-http transport and an operator token; stdio deployments and servers without that token are unaffected.
