Server crash via unbounded nonce cache

Resource exhaustion in Sterfive SAS node-opcua before 2.166.0 allows remote attackers to crash servers via repeated session creation. The process-global g_alreadyUsedNonce object records nonces from OpenSecureChannelRequest and CreateSession without expiration or a size limit. CreateSession requires no certificate, and unique nonces persist across expired sessions until heap exhaustion terminates the Node.js process.
